Would you like to use a school district building, school yard, gym, auditorium or other district facility, outside of the regular school day? (For example, before or after school, on a weekend or during a school vacation.) Yes

(Note: If you will be paid by the SFUSD to provide services to students, staff or families, you will need a CONTRACT with the SFUSD. Please speak with your contact person at the school district related to these services, because contracts need to be generated by SFUSD staff.)

Form Guidance

You will need a FACILITIES USE PERMIT.

You do not need a separate Memorandum of Understanding (MOU) with the SFUSD. Please coordinate directly with the ExCEL site coordinator or Beacon program director.

You will need a MEMORANDUM OF UNDERSTANDING (MOU) with the SFUSD. Please select “MOU Requests” in the menu after you log into your account.
