ASM Summer 2018 Program Staff
Please encourage your teens to check for their Ventra information and sign the PACR as you contact them to confirm enrollment for this summer!
• Ventra information – After School Matters will help pay for participating teens’ transportation on CTA buses and trains for the first two weeks of summer programs. To be eligible for this transit support benefit, teens must provide their Ventra card number and expiration date in the summer application by Thursday, June 7, 2018. Learn more.
• PACR signatures – After School Matters is rolling out a revised Program Acknowledgment, Consent, and Release (PACR) Form for Summer 2018. ALL TEENS in an apprenticeship program-whether returning or new to programs-must sign a PACR form to participate and be eligible to receive a stipend. The PACR must be read and signed by both the teen (including those 18 and older) AND a parent or guardian.
Encourage teens and parents to check their email inboxes for the After School Matters Application Submitted email subject line. In the email, there is a highlighted section directing parents to “ …click here to e-sign the Program Acknowledgment, Consent, and Release (PACR) for your child.”
• Can’t find the email? Ask parents/guardians to search thier inbox and/or spam email folders for “Parent PACR” to easily find the email that contains the link to e-sign the PACR for their teen.
Teens and parents/guardians can review the Teen Pay & Stipends page of their application to see if they have completed the summer checklist!

Field Trip Forms:
ASM program participants, including those 18 and older, must get their parents' or guardians' written permission to be eligible to attend a field trip. There is one exception to this: unaccompanied youth (youth who are not in the physical custody of a parent or guardian) who are 18 or older may provide written consent on their own behalf.PACR:
Starting in Summer 2017, teens who are new to ASM, including those 18 and older, must obtain their parents or guardians signature on their Program Acknowledgment, Consent, and Release Form. Make sure your e-mail address is correct! Click on Participants & Staff on the left menu and then click on Program Staff. If your e-mail address is wrong, click your name to edit it. If you see that a colleague's e-mail address is wrong, let him/her know to log in and correct it.
